March 5, 2018 – A Letter of Acceptance, Waiver and Consent (AWC) was issued in which Andrews was assessed a deferred fine of $5,000 and suspended from association with any FINRA member in all capacities for two months. Without admitting or denying the findings, Andrews consented to the sanctions and to the entry of findings that he willfully failed to timely amend his Form U4 to disclose a federal tax lien filed against him for $13,752. The suspension was in effect from March 5, 2018, through May 4, 2018. (FINRA Case #2016050192001).
